My board space was limited, so I used the Maxim 1794 DC-DC converter. It is in a SOT package, and even has it's own zero crossing detector switch, which eliminated the Schottky diode. It will start at .8 VDC and draws only a few microamps when not loaded down. Check it out...

I believe the correct part number is MAX1724.
